RN Care Manager

Accepting applications for a full-time RN Care Manager at its Madawaska Community Health Center. The ideal candidate must be professional, reliable, compassionate, self-motivated, detailed-oriented, mission-minded, have excellent communication and customer service skills, and the ability to multi-task in a fast-pace clinical office setting. The primary responsibility of this position will be to assist in population health and chronic care management, care coordination, Community Care Team initiatives, Medicare Wellness visits, community outreach, office nurse coverage and other initiatives related to patient care and behavioral health management services. Experience with an Electronic Health Record system is highly preferred. Computer skills and knowledge required. Ability to speak French helpful, but not required. Ability to travel between practice sites, patient homes, other offsite locations, and other work related trainings required.
Minimum Qualifications Required: Baccalaureate degree in the field of nursing and possess an active/valid Maine nursing and driver's license. Nursing experience in a health care setting or community health field preferred.


Licensed Clinical Social Worker
NHSC Loan Repayment Eligible

Accepting applications for a full-time Licensed Clinical Social Worker to join our patient-centered medical home. LCSW will work with patients in providing behavioral health counseling services. The practitioner in this role will collaborate in an integrated model of care with Primary Care Providers to recognize, treat and manage behavioral health and psychosocial issues while acting as a contributing member to the Integrated Primary Care Team. A genuine interest to primarily provide high quality care to adolescent population, as well as all ages.
Minimum Qualifications Required: Must be a Licensed Clinical Social Worker with the State of Maine. The candidate must also possess strong communications skills, ability to work in primary care as part of an integrated team, ability and means to travel between service delivery sites.


Referral Specialist/Medical Record Technician

Accepting applications for a full-time Referral Specialist/Medical Record Technician at its Eagle Lake Health Center. The ideal candidate must be professional, reliable, compassionate, self-motivated, detailed-oriented, mission minded, have strong verbal communication skills and telephone etiquette, have excellent customer service skills, and the ability to multi-task in a fast-pace office setting. The Referral Specialist/Medical Record Technician will be responsible for patient referrals, data entry, barcoding/scanning information, etc. in an Electronic Health Record [EHR] system for FRRH's practice sites. Experience with an EHR system is highly preferred. Computer skills specifically Microsoft Word and Excel and experience with data registries, insurance portals, tracking logs, etc. preferred. Ability to travel between sites required.

Minimum Qualifications Required: High School Diploma/GED required. Experience in a clinical practice office, preferred. Ability to speak French helpful, but not required.
