Full Time Case Manager
Case Management; Aroostook County

The Case Manager provides case management services to adults and children with Intellectual Disabilities and/or Mental Illness.

Essential Duties:
Works with parents/legal guardians, community support workers, program coordinators, therapists, and other health professionals in developing and implementing a person centered plan
Provides direct treatment services on an as needed basis
Documents client's progress in accordance with established treatment plan
Coordinates service planning meetings and facilitates sharing of information, level of functioning, and progress of consumers in services
Assists with development of crisis plans for consumers in need

Job Specifications:
Bachelor's degree with a concentration in social work or related field
Must demonstrate ability to understand and relate positively to individuals with intellectual disabilities and/or mental illness
Must be highly motivated and well organized
Must be able to function independently
Must be able to write in a clear, concise manner
Must exhibit proficient computer skills
Must have valid driver's license and good driving record; must be able to travel throughout Aroostook County
